    Relationship between patients with alcoholic intoxication due to alcoholic drinks and the presence of hypoglycemia

    Introduction: Alcoholic intoxication is a frequent cause of patient’s medical care in recreation and enjoyment areas. Numerous are complications associated to this clinic syndrome, being described among them the hypoglycemia, intending in their treatment a customary use of glucose solutions. Objective: To determine the hypoglycemia incidence in patient with intoxication by alcoholic drinks in a recreational area "East Beach" touristic area. Material and Methods: A descriptive study was performed in patients hospitalized in an emergency service due to alcoholic intoxication and to who was carried out a capillary glycaemia analysis to reveal the presence of hypoglycemia. 59 patients with alcoholic poisoning were included in the study, without significant differences of gender, being young people and adolescents most of cases. Results: Most frequent symptoms were a level of conscience deterioration, predominating in the 62,7 % a Glasgow Coma Scale ≤ 9. 66,2 % did not ingest food before their admission. Vomiting and the dehydration was present in 62,7 and 69,4 respectively of the hospitalized patients. The transfer was necessary in 15,2 % to other hospitals to continue the studies and care. Only hypoglycemia was found in four cases for a 6,7 %, prevailing in patients older than 40 years. Conclusions: The hypoglycemia can be present in the alcoholic poisoning, being very little probable in young people and adolescents, it is related mainly to scarce food ingestion.Keywords: Poisoning, Alcoholic Drinks, Emergency Services, Hypoglycemia, Hypertonic Glucose, signs, symptoms.</p

    Manejo de casos sospechosos de la nueva gripe AH1N1 por los servicios de emergencias móviles.

    Los servicios de emergencias móviles son un eslabón clave en la clasificación, y manejo de casos sospechosos de nueva gripe AH1N1. Los mismos garantizan una racional asistencia dependiendo de la necesidad de la atención que requiera cada caso. Nuestro objetivo es mostrar la organización creada para la asistencia de estos pacientes. Se realizó una revisión de los protocolos propuestos por la organización mundial de la salud, el Centro de Control de Enfermedades de Atlanta y otras bibliografías nacionales sobre el manejo de pacientes ante epidemias. Se trataron los casos a partir de los hallazgos clínicos, complementarios y factores de riesgos, mejorando la clasificación en los servicios de urgencias y su traslado a centros adecuados para su atención. La clasificación adecuada de casos, elimina el margen de error en la coordinación, y garantiza una asistencia adecuada y traslado al centro con condiciones para el tratamiento de sus posibles complicaciones.  Palabras clave: Nueva gripe, servicios de emergencias móviles, móviles básicos, móviles avanzados

    Guideline for teaching pre-hospital life support. Consensus for the project of training and research in life support. Cienfuegos 2018

    Foundation: In the pre hospital scenario mortality due to emergencies is high, mainly because episodes are produced before arriving to the health system. Health personnel should be trained for facing these emergencies in this context. The project for training and researching in life support in emergencies and disasters nay be an academic framework to achieve it.Objective: to update the Project, guidelines and teaching strategy for training life support before emergencies in pre hospital emergencies.Method: qualitative study which had as a starting point the development of a National Workshop with 8 experts and 6 key informers, on July 10th and 11th in Cienfuegos. Besides documentary review, there were applied the techniques of brainstorming, nominal group, semi structured questionnaire.Results: the elements proposed for teaching pre-hospital life support, in under graduate and postgraduate courses were the following: objectives, didactical elements, teaching strategy, content, scope, skills to be developed, future research and areas of inter institutional collaboration. It was defined that the structure of a course model for teaching the topic and the main considerations for its execution.Conclusion: academic proposals for life support contain basic elements for training health professionals and students to provide medical assistance at site and in institutions of firs level of health.</p

Múltiples son las complicaciones asociadas a este síndrome clínico; se describen entre ellas la hipoglucemia y se propone en su tratamiento el uso rutinario de soluciones glucosadas para tratarla. Objetivo: Determinar la incidencia de hipoglucemia en pacientes con intoxicación por bebidas alcohólicas en un área recreativa del polo turístico de Playas del Este. Material y Métodos: Se realizó un estudio descriptivo en pacientes ingresados por intoxicación alcohólica en un servicio de emergencias, a los que se les realizó glicemia capilar al ingreso analizándose la presencia de hipoglucemia. Se incluyeron en el estudio 59 pacientes con intoxicación alcohólica, sin diferencias significativas en cuanto al sexo; se evidenció un mayor número de casos intoxicados en los jóvenes y adolescentes. Resultados: Los signos más frecuentes al ingreso fueron el deterioro del nivel de conciencia predominando el Glasgow ≤ 9 en 62,7%. Un 66,2% no había ingerido alimentos previos al ingreso. El vómito y la deshidratación se presentaron en 62,7 y 69,4% de los pacientes ingresados. El traslado a otros servicios médicos fue necesario en 15,2% a servicios hospitalarios para continuidad de estudios y cuidados. Solo se encontró hipoglucemia en cuatro casos para 6,7%; predominó en pacientes mayores de 40 años. Conclusiones: La hipoglucemia puede estar presente en la intoxicación alcohólica, siendo muy poco probable en los jóvenes y adolescentes; se asocia mayormente a la escasa ingesta de alimentos

Was carried out a revision of the protocols proposed by the World Health Organization, the Centers for Disease Control and Prevention of Atlanta and other national bibliographies on the Management of patient before epidemics. The cases were managed starting from the clinical, complementary discoveries and factors of risks, improving the classification in the urgencies services and their transfer to appropriate centers for their attention. The appropriate classification of cases, eliminates the error margin in the coordination, guaranteeing an appropriate attendance and transfer to the center with conditions for the treatment of its possible complications
